Summary

Extramedullary plasmacytoma, a rare plasma cell tumor, can be extremely aggressive. This case highlights rapid metastasis to the brain and rib, leading to a fatal outcome during initial chemotherapy.

Background:

  • Extramedullary plasmacytoma (EMP) is a rare plasma cell neoplasm.
  • It typically presents as a localized tumor outside the bone marrow.

Observation:

  • A 56-year-old male presented with epistaxis and anemia.
  • Nasendoscopy revealed a right maxillary sinus mass extending into the nasopharynx.
  • MRI confirmed tumor extension to nasal cavity, sphenoid/ethmoidal sinuses, brain, and rib.

Findings:

  • Histopathology confirmed extramedullary plasmacytoma with kappa-chain restriction.
  • The tumor demonstrated extremely aggressive behavior with widespread metastasis.
  • The patient succumbed to the disease during the first cycle of chemotherapy.
