  • Outcomes assessed included symptom relief, scar characteristics, return to hand use, and strength.
  • Main Results:

    • Both techniques showed high rates of symptom resolution (82.4% vs 88.2%).
    • The distal incision group experienced significantly less scar rigidity and sensitivity (11.8% vs 52.9%).
    • Patients with distal incisions returned to basic daily activities (9 days vs 18 days) and normal function (21 days vs 35 days) significantly faster.
    • No significant differences were observed in grip and pinch strength recovery between groups.

    Conclusions:

    • Both mini incision techniques for carpal tunnel release provide satisfactory mid- and long-term results.
    • The mini skin incision on the distal side of the transverse carpal ligament is associated with reduced incision-related morbidity.
    • Distal mini incisions facilitate quicker functional recovery and less scar tissue issues.
