Intraoperative electron beam radiation therapy for retroperitoneal soft tissue sarcoma

Background:

Purpose of the Study:

• To evaluate the efficacy of combined external beam radiation therapy (EBRT), surgical resection, and intraoperative radiation therapy (IORT) for retroperitoneal sarcoma.
• To assess local control, disease-free survival, and overall survival rates in patients treated with this multimodal approach.

Main Methods:

• A retrospective analysis of 20 patients with primary or recurrent retroperitoneal sarcoma treated between 1981 and 1989.
• Patients received 4000–5000 cGy of EBRT followed by surgical resection and, in select cases, IORT.
• Surgical outcomes included complete or partial resection, and IORT was administered for microscopic or gross residual disease.

• Of 17 patients who underwent resection, the 4-year actuarial local control and disease-free survival rates were 81% and 64%, respectively.

Conclusions:

• Aggressive multimodal therapy, including EBRT, surgical resection, and IORT, appears to provide satisfactory resectability and local control for retroperitoneal sarcoma.
• The combination of these treatment modalities demonstrates acceptable tolerance and offers a viable option for managing these challenging tumors.
