Background:

Phenylketonuria (PKU) is a rare genetic disorder affecting amino acid metabolism.
Early diagnosis and dietary management are critical to prevent intellectual disability.

Observation:

A female patient with PKU and mild intellectual disability developed behavioral issues and anorexia nervosa at age 14.
Anorexic symptoms resolved by age 20, but behavioral disorder escalated, necessitating psychiatric hospitalization.

Findings:

Serum phenylalanine levels may correlate with the progression of behavioral disorders in PKU patients.
The interplay between metabolic control and psychiatric symptoms in PKU requires further investigation.

Implications:

Highlights the potential for psychiatric comorbidities in individuals with PKU.
Suggests the need for integrated care models addressing both metabolic and mental health aspects of PKU.
