Background:

Pancreatic pseudocysts are common complications of pancreatitis.
Large symptomatic pseudocysts often require drainage.
Endoscopic drainage is a minimally invasive treatment option.

Purpose of the Study:

To evaluate the efficacy of percutaneous transgastric cystgastrostomy as a single-step procedure.
To assess the short-term outcomes of this technique in patients with large pancreatic pseudocysts.

Main Methods:

Retrospective analysis of 12 patients undergoing single-step percutaneous transgastric cystgastrostomy.
Patients treated between 2002 and 2007 with follow-up data up to 1 year.
Drainage achieved using one or two stents placed during the procedure.

Main Results:

Successful completion of the procedure in all 12 patients.
Complete pseudocyst resolution in 7 of 12 patients (stent passage or endoscopic removal).
Complete resolution in 2 of 12 patients with stents in situ; 2 infected pseudocysts required surgery; 1 partial resolution with reaccumulation.

Conclusions:

Percutaneous transgastric cystgastrostomy is a highly successful single-step procedure.
It offers good short-term outcomes for large symptomatic pancreatic pseudocysts.
This technique should be considered for managing such cases.
