Appendicitis01:19

Appendicitis

Appendicitis is an acute inflammatory condition of the vermiform appendix, most commonly caused by obstruction of its lumen. The appendix is a narrow, blind-ended pouch that extends from the cecum, making it particularly prone to obstruction. Causes include fecaliths, lymphoid hyperplasia (often after viral infections), parasites, tumors, or foreign bodies. This obstruction initiates a cascade of pathological changes.Luminal Obstruction and Early InflammationAfter obstruction, normal mucosal...
Assessment of the Rectum and Anus01:25

Assessment of the Rectum and Anus

Evaluating the rectum and anus plays a crucial role in conducting a thorough physical examination of the gastrointestinal system. Although it may be uncomfortable and often embarrassing for the patient, it holds immense diagnostic value, particularly in detecting gastrointestinal diseases and abnormalities. This guide will explain how to perform this assessment using inspection and palpation methods.
Rectal Inspection
Begin by inspecting the perianal and anal areas for color, texture, rashes,...
Inflammatory Bowel Disease V: Surgical Management01:21

Inflammatory Bowel Disease V: Surgical Management

Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Summary

Extramammary Paget's disease (EMPD) of the perianal region is rare and often linked to other cancers. Surgical excision with reconstruction showed good outcomes, though local recurrence is possible, necessitating long-term monitoring.

Area of Science:

  • Oncology
  • Dermatology
  • Surgical Pathology

Background:

  • Extramammary Paget's disease (EMPD) is a rare neoplasm.
  • It is frequently associated with underlying cutaneous or visceral malignancies.
  • Perianal EMPD presents unique diagnostic and management challenges.

Observation:

  • This retrospective study analyzed 5 cases of perianal EMPD treated between 1996 and 2006.
  • Treatments included wide perianal excision, reconstruction with cutaneous grafts, and transanal resection for rectal adenocarcinoma.
  • Temporary sigmoid stomas were used in two cases to aid wound healing.

Findings:

  • No immediate or long-term complications were recorded.
  • Two patients experienced local recurrence; none developed distant metastases.
  • Four patients remain disease-free, while one with multiple recurrences died from unrelated heart failure.

Implications:

  • Perianal EMPD management requires a multidisciplinary approach.
  • The association with other malignancies necessitates thorough investigation and long-term follow-up.
  • Despite radical surgery, the potential for recurrence and metastasis highlights the complexity of this disease.
