Background:

  • Clinical islet transplantation is a promising therapy for type 1 diabetes but faces donor organ scarcity.
  • Donor selection criteria often exclude individuals with mild hyperglycemia, potentially limiting the islet supply.

Observation:

  • Two cases of islet isolation from donors with elevated hemoglobin A1c (HbA1c) levels (6.3% and 7.9%) are presented.
  • Adequate islet isolation yields were achieved, yielding 521,350 and 497,472 islet equivalents.
  • Graft analysis of the higher HbA1c donor showed a reduced beta-cell proportion (21.6%) and increased alpha-cell proportion (51.0%).

Findings:

  • Both islet preparations were successfully transplanted into recipients with brittle type 1 diabetes.
  • One recipient achieved insulin independence for 4 years with good glycemic control.
  • The other recipient, receiving islets from the higher HbA1c donor, experienced a 56% reduction in insulin requirements.

Implications:

  • Pancreases from donors with mild hyperglycemia represent a potential, previously underutilized source for islet isolation.
  • This expands the donor pool for clinical islet transplantation, offering hope for more type 1 diabetes patients.
  • Further research into the long-term function and characteristics of islets from hyperglycemic donors is warranted.
