Summary

Low-dose chemotherapy, including methotrexate and vinblastine, effectively managed extra-abdominal desmoid tumors, stabilizing disease and alleviating pain. While toxicities were noted, this approach proved valuable for symptom improvement and preventing progression.

Background:

  • Desmoid tumors, though rare, can cause significant morbidity.
  • Extra-abdominal desmoid tumors present unique treatment challenges.
  • Low-dose chemotherapy is explored as a therapeutic option.

Purpose of the Study:

  • To evaluate the efficacy of low-dose chemotherapy (methotrexate and vinblastine) for extra-abdominal desmoid tumors.
  • To assess tumor response and treatment-related toxicity in patients with desmoid tumors.
  • To determine the impact of chemotherapy on patient symptoms and quality of life.

Main Methods:

  • Retrospective review of 12 patients with extra-abdominal desmoid tumors treated between 1996-2003.
  • Weekly administration of low-dose methotrexate and vinblastine.
  • Evaluation of tumor response, symptom relief, functional improvement, and treatment toxicity.

Main Results:

  • Partial response or stable disease observed in all patients (6 partial response, 6 stable).
  • Significant symptom relief reported in 6 of 7 patients with painful tumors.
  • Treatment-related toxicities were common, though generally manageable; peripheral neuropathy was the main long-term side effect.

Conclusions:

  • Low-dose chemotherapy is an effective strategy for managing extra-abdominal desmoid tumors.
  • This treatment modality helps prevent local disease progression.
