Purpose of the Study:

  • To assess the incidence of malignancies in type 2 diabetic patients.
  • To evaluate the association between different sulphonylureas and cancer risk.
  • To identify potential differential effects of hypoglycemic drugs on cancer development.

Main Methods:

  • A matched case-control study design was employed.
  • 195 patients with incident malignancy (cases) were matched with 195 cancer-free diabetic patients (controls).
  • Exposure to hypoglycemic drugs, particularly sulphonylureas, was assessed over a 10-year period preceding the event.

Main Results:

  • Long-term exposure (>36 months) to metformin (adj. OR 0.28) and gliclazide (adj. OR 0.40) was associated with a significantly reduced risk of cancer.
  • Conversely, glibenclamide use for at least 36 months was linked to an increased incidence of malignancies (adj. OR 2.62).
  • No significant association between cancer incidence and treatment with insulin, thiazolidinediones, or acarbose was observed.

Conclusions:

  • Individual sulphonylureas may exert differential effects on cancer risk in type 2 diabetic patients.
  • Gliclazide may offer a protective effect against cancer, while glibenclamide might increase malignancy risk.
  • Further research is warranted to confirm these findings and elucidate the underlying mechanisms.
