Summary

This study investigated upper aerodigestive tract infections in patients with cervical tuberculous lymphadenitis (TBLN). Six percent of patients had tuberculous nasopharyngitis, but other upper airway infections were not found.

Background:

  • Cervical tuberculous lymphadenitis (TBLN) is a significant global health issue.
  • The primary source of infection for TBLN is often unidentified.
  • Associated infections in the upper aerodigestive tract (UAT) require further investigation.

Purpose of the Study:

  • To prospectively identify associated mycobacterial infections in the UAT of patients diagnosed with cervical TBLN.
  • To determine the prevalence of tuberculous nasopharyngitis in this patient cohort.

Main Methods:

  • A prospective study included 113 patients with suspected cervical TBLN.
  • Physical examination of the UAT, routine endoscopy, and nasopharyngeal biopsy were performed.
  • Cervical lymph node biopsies were analyzed via histology and culture for Mycobacterium tuberculosis.

  • Histological confirmation of cervical TBLN was found in 75 patients (66%).
  • Mycobacterium tuberculosis was identified in 90% of confirmed cervical TBLN cases.

  • While tuberculous nasopharyngitis can occur with cervical TBLN, it is relatively uncommon.
  • The primary source of mycobacterial infection remains elusive in a significant proportion of cervical TBLN cases.
  • Further research is needed to elucidate the transmission dynamics and primary foci of TBLN.
