Area of Science:

  • Ophthalmology
  • Visual Science

Background:

  • Nonorganic visual loss (NVL) presents diagnostic challenges.
  • Accurate diagnosis of NVL is crucial for appropriate patient management.

Purpose of the Study:

  • To evaluate the diagnostic value of the distance doubling visual acuity test for nonorganic visual loss.
  • To compare the test's performance against organic visual loss controls.

Main Methods:

  • A comparative observational case series included 21 patients with NVL and 21 controls with organic visual loss.
  • Best corrected visual acuity was measured using Landolt Cs at 5 meters and then at 10 meters via a mirror.

Main Results:

  • The distance doubling visual acuity test correctly identified NVL in all 21 patients.
  • The test demonstrated 100% sensitivity (CI; 83%-100%) and 100% specificity (CI; 82%-100%) for functional visual loss.

Conclusions:

  • The distance doubling visual acuity test is a valuable tool for diagnosing nonorganic visual loss.
  • The test exhibits high sensitivity and specificity, confirming its utility in detecting nonorganic visual impairment.
