Summary

Patients with end-stage kidney disease have a high risk of bilateral kidney tumors. Careful monitoring and consideration of contralateral nephrectomy are recommended, especially for transplant recipients on immunosuppression.

Background:

  • Patients with end-stage kidney disease (ESKD) have an increased risk of renal cell carcinoma (RCC).
  • ESKD-associated RCC often presents as multifocal and bilateral tumors.
  • The optimal surgical management for unilateral RCC in ESKD patients remains unclear.

Purpose of the Study:

  • To investigate the occurrence of bilateral and multifocal renal tumors in patients with ESKD.
  • To evaluate the clinical outcomes of nephrectomy in ESKD patients with kidney tumors.
  • To inform surgical decision-making regarding unilateral versus bilateral nephrectomy in this population.

Main Methods:

  • Retrospective review of 14 ESKD patients diagnosed with kidney tumors between January 2002 and December 2006.
  • Analysis of surgical procedures, including unilateral and bilateral nephrectomies.
  • Histopathological examination of resected specimens to determine tumor characteristics.

Main Results:

  • Kidney tumors were found in all 19 specimens from 14 patients.
  • Tumors were multiple in 13 kidneys (68.4%) and solitary in 6 (31.6%).
  • Papillary RCC (PRCC) was the most common type (68.4%), followed by clear cell RCC (CRCC) (47.4%). One patient had myxoid liposarcoma with contralateral PRCC.

Conclusions:

  • There is a significant risk of bilateral kidney tumor involvement in ESKD patients.
  • Regular follow-up is crucial for patients undergoing unilateral nephrectomy.
  • Contralateral nephrectomy should be carefully considered, particularly in immunosuppressed transplant recipients, but prophylactic contralateral nephrectomy is not currently standard practice.
