Summary

Interfacial gaps between femoral stems and bone in cementless hip replacements have minimal impact on mechanical stability. Finite element analysis shows micromotion is not significantly affected by gap size or location in most cases.

Area of Science:

  • Biomechanical Engineering
  • Orthopedic Surgery
  • Materials Science

Background:

  • Cementless total hip arthroplasty frequently results in interfacial gaps between the femoral stem and bone.
  • The influence of these gaps on the mechanical stability of the femoral stem remains inadequately understood.

Purpose of the Study:

  • To quantify the effect of interfacial gaps on the primary mechanical stability of a Versys Fiber Metal Taper femoral stem.
  • To analyze stem micromotion under simulated stair climbing loads using finite element modeling.

Main Methods:

  • Development of a finite element model incorporating various interfacial gap definitions between the femoral stem and bone.
  • Simulation of 500 random interfacial gap configurations to assess micromotion.
  • Simulation of 17 gap configurations based on experimentally observed patterns.

Main Results:

  • Stem micromotion was inversely proportional to the bone-implant contact ratio, with greater variance at lower contact ratios.
  • Gaps in the proximal-medial region significantly influenced micromotion.
  • For contact ratios exceeding 40%, micromotion showed minimal change, suggesting limited clinical impact from typical gap variations.

Conclusions:

  • Variations in interfacial gap size and location are unlikely to cause significant differences in micromotion for this cementless stem.
  • The effect of interfacial gaps on micromotion is likely overshadowed by other clinical factors.
  • Interfacial gaps are not expected to have a noticeable effect on the clinical micromotion of cementless femoral stems.
