Treatment Resistant Cancers02:56

Treatment Resistant Cancers

Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Psychosis: Goals of Pharmacotherapy

Antipsychotic drugs are a crucial treatment method for acute and chronic psychoses, bipolar illness, and behavioral disorders. The selection of these drugs depends on several factors, including the state of the disease, clinical judgment, possible drug interactions, and the patient's sensitivity to adverse effects. In immediate scenarios, such as delirium and dementia, short-term treatment with low doses of high-potency typical or atypical agents can effectively manage symptom exacerbation. For...

Treatment Strategies for Psychological Disorders01:24

Treatment Strategies for Psychological Disorders

Treatment approaches for psychological disorders fall into three main categories: psychological, biological, and sociocultural. Each approach targets different aspects of mental health, requiring varying levels of education and training.
Psychological therapies focus on modifying emotions, thoughts, and behaviors through talking, interpreting, listening, rewarding, challenging, and modeling. Clinical psychologists, counselors, and social workers commonly practice psychotherapy. Clinical...
Drug Therapy01:28

Drug Therapy

The advent of drug therapy has profoundly shaped modern mental health care, providing targeted treatments for a range of psychological disorders. Psychotherapeutic drugs, classified into antianxiety, antidepressant, and antipsychotic medications, address symptoms across anxiety disorders, mood disorders, and schizophrenia. While these medications have transformed patient outcomes, they require careful management due to their potential side effects and limitations.

Inflammatory Bowel Disease IV: Pharmacological Management

Upon diagnosis, managing Inflammatory Bowel Disease (IBD) involves addressing several crucial aspects. The primary goals include resting the bowel, correcting malnutrition, and providing symptomatic relief. Resting the bowel may consist of medications to reduce inflammation and promote healing. Correcting malnutrition is essential, often requiring dietary adjustments and nutritional supplements. Symptomatic relief aims to ease pain, diarrhea, and other discomforts in IBD.

Summary

This case study highlights lupus vulgaris, a rare form of cutaneous tuberculosis, initially misdiagnosed as psoriasis. Prompt diagnosis and multi-drug antitubercular treatment led to rapid resolution of skin lesions.

Area of Science:

  • Dermatology
  • Infectious Diseases
  • Microbiology

Background:

  • Cutaneous tuberculosis presents diverse clinical manifestations.
  • Lupus vulgaris is the most common form of cutaneous tuberculosis.
  • Accurate diagnosis can be challenging, often mimicking other dermatoses like psoriasis.

Observation:

  • A 61-year-old male with a 25-year history of psoriasis vulgaris developed a nodule within an erythematous plaque.
  • Biopsy revealed epithelioid cell granulomas with Langhans giant cells, excluding squamous cell carcinoma.
  • Positive tuberculin test and PCR detection of Mycobacterium tuberculosis complex confirmed tuberculosis.

Findings:

  • The patient was diagnosed with lupus vulgaris, a common form of cutaneous tuberculosis.
  • Histopathological findings of granulomas with giant cells are characteristic of tuberculosis.
  • Molecular detection (PCR) confirmed Mycobacterium tuberculosis complex in biopsy samples.

Implications:

  • This case highlights the importance of considering infectious causes, like tuberculosis, in recalcitrant or evolving skin lesions initially diagnosed as psoriasis.
  • Accurate diagnosis through biopsy, histopathology, and molecular methods is essential for appropriate management.
  • Prompt antitubercular treatment led to rapid resolution of lupus vulgaris, underscoring the efficacy of standard regimens.
