Background:

  • Isolated nodular infarction is a rarely described clinical entity in humans.
  • This study aims to detail the clinical and laboratory findings of isolated nodular infarction.

Observation:

  • Eight patients with isolated nodular infarction underwent comprehensive neurotological evaluation and neuroimaging.
  • Patients presented with isolated vertigo and significant imbalance, often mimicking peripheral vestibulopathy.
  • Common findings included unilateral nystagmus, ipsilesional falling, and normal head impulse and caloric tests.

Findings:

  • Six patients had unilateral lesions, and two had bilateral lesions.
  • Abnormal vestibular findings included periodic alternating nystagmus, impaired tilt suppression, and positional nystagmus.
  • MR angiography revealed ipsilesional vertebral artery hypoplasia in three patients.

Implications:

  • Isolated nodular infarction presents with vertigo and imbalance, crucial to differentiate from peripheral vestibular disorders.
  • Severe imbalance and negative head impulse tests are key discriminators.
  • Findings suggest disruption of vestibular signal processing and the velocity storage mechanism.
