Area of Science:

  • Thoracic Surgery
  • Minimally Invasive Procedures
  • Surgical Oncology

Background:

  • Endoscopic thoracic surgery is evolving.
  • Traditional thoracotomy has limitations.
  • Cost-effectiveness of minimally invasive techniques is often debated.

Purpose of the Study:

  • To present the initial experience with purely endoscopic anatomical lung resection (lobectomy with mediastinal lymphadenectomy).
  • To describe the surgical techniques and tactics employed.
  • To compare endoscopic procedures with conventional open surgery and assess their financial implications.

Main Methods:

  • Five purely endoscopic lung resections were performed: one left superior lobectomy and four left inferior lobectomies.
  • Procedures utilized modern stapling devices and haemoclips.

  • Surgical tactics for thoracoscopic operations were documented.
  • Main Results:

    • Successful completion of five purely endoscopic lung resections.
    • Detailed description of surgical procedures and tactics.
    • Data collected for comparison with open thoracotomy, addressing cost perceptions.

    Conclusions:

    • Purely endoscopic anatomical lung resection is a viable technique.
    • The procedure can be performed effectively using current endoscopic tools.
    • The financial intensity of endoscopic thoracic surgery may be less than commonly believed.
