    Summary

    Lung carcinoid and oat cell tumors synthesize adrenocorticotropic hormone (ACTH)-like materials, even without clinical ectopic ACTH syndrome. Normal lung tissue in cancer patients also shows ACTH, suggesting widespread production or tumor secretion.

    Background:

    • Ectopic ACTH syndrome is a rare complication of certain cancers.
    • Lung tumors, particularly carcinoid and oat cell types, are known to produce hormones.

    Purpose of the Study:

    • To investigate adrenocorticotropic hormone (ACTH) production in lung tumors.
    • To correlate ACTH levels with tumor type and ultrastructural findings.
    • To examine ACTH levels in normal lung tissue from cancer patients.

    Main Methods:

    • Measurement of ACTH levels using N- and C-terminal immunoassays and cytochemical bioassay.
    • Analysis of fourteen lung tumors and corresponding normal lung tissue.
    • Ultrastructural examination for secretory granules.

    • Significant immunoactive and bioactive ACTH concentrations were found in carcinoid, oat cell, combined, and poorly differentiated squamous cell tumors.
    • Carcinoid tumors showed higher C-terminal than N-terminal ACTH immunoactivity.
    • Normal lung tissue from cancer patients contained ACTH-like material, correlating with tumor ACTH levels.

    Conclusions:

    • Lung carcinoid and oat cell tumors synthesize ACTH-like materials, irrespective of clinical ectopic ACTH syndrome.
    • The presence of ACTH in non-tumorous lung tissue may indicate low-level production or tumor-secreted granule sequestration.
