Background:

Mitral valve prosthesis thrombosis is a serious complication.
Limited consensus exists on optimal therapeutic strategies.
Immobility of a valve leaflet poses a significant risk.

Purpose of the Study:

To report a successful case of fibrinolysis for acute mitral valve prosthesis thrombosis.
To discuss therapeutic strategies for mitral valve thrombosis.
To highlight the role of recombinant tissue plasminogen activator (rTPA) in selected cases.

Main Methods:

A 42-year-old male patient with acute mitral valve prosthesis thrombosis and immobile emidisk.
Treatment with recombinant tissue plasminogen activator (rTPA).
Evaluation of thrombus size and patient contraindications.

Main Results:

Successful resolution of mitral valve prosthesis thrombosis with rTPA.
Demonstration of a viable fibrinolytic strategy in a specific clinical scenario.
Thrombus dimension was less than 1 cm, favoring fibrinolysis.
