Intrauterine insemination (IUI) failure in idiopathic subfertility is not linked to diminished fertilization. Subsequent in vitro fertilization (IVF) success rates are similar whether or not patients previously underwent IUI.
Area of Science:
- Reproductive Medicine
- Infertility Research
- Assisted Reproductive Technologies
Background:
- Idiopathic subfertility affects couples without a clear cause.
- Intrauterine insemination (IUI) is a common first-line treatment.
- Understanding IUI failure mechanisms is crucial for optimizing fertility treatments.
Purpose of the Study:
- To investigate if fertilization issues contribute to IUI failure in idiopathic subfertility.
- To compare fertilization rates in initial in vitro fertilization (IVF) after failed IUI versus immediate IVF.
Main Methods:
- A secondary analysis of a randomized controlled trial comparing IUI and IVF.
- Compared total fertilization failure (TFF) rates in first IVF cycles.
- Included patients with idiopathic subfertility who had either 6 cycles of unsuccessful IUI or underwent immediate IVF.
Main Results:
- 45 patients underwent IVF post-IUI failure; 58 underwent immediate IVF.
- Total fertilization failure (TFF) occurred in 5% after IUI vs. 13% in immediate IVF groups.
- After adjusting for confounders, TFF rates did not significantly differ (p=0.08).
Conclusions:
- Fertilization rates in the first IVF cycle are comparable regardless of prior IUI treatment in idiopathic subfertility.
- Impaired fertilization is unlikely to be a primary reason for IUI treatment failure.
- This suggests other factors influence IUI success in idiopathic subfertility cases.
