Summary

This study on liver transplant recipients found acute rejection in 58.7% of patients. While most acute rejections were treated successfully, increased rejection treatments correlated with a higher risk of death.

Steroid-Responsive Acute Rejection Should Not Be the End Point for Immunosuppressive Trials.

American journal of transplantation : official journal of the American Society of Transplantation and the American Society of Transplant Surgeons·2016

Area of Science:

  • Transplantation Immunology
  • Hepatology
  • Clinical Medicine

Background:

  • Liver transplantation is a life-saving procedure for end-stage liver disease.
  • Immunosuppression is crucial to prevent allograft rejection.
  • Optimizing immunosuppression protocols is essential for long-term graft survival.

Purpose of the Study:

  • To evaluate the incidence and outcomes of acute rejection in primary liver allograft recipients.
  • To assess the efficacy of rejection treatment and its impact on patient survival.
  • To analyze the relationship between rejection treatment frequency and mortality risk.

Main Methods:

  • Retrospective analysis of 138 primary liver allograft recipients.
  • Standard immunosuppression regimen: cyclosporine and prednisolone with azathioprine induction.
  • Biopsy-confirmed clinical rejection as the primary endpoint for rejection diagnosis.

Main Results:

  • Acute rejection occurred in 58.7% of patients.
  • Treatment for acute rejection was successful in 88.9% of cases.
  • Rejection-related death occurred in 4.3%, and retransplantation in 2.2% of patients.

Conclusions:

  • Acute rejection is common in liver transplant recipients on this immunosuppression protocol.
  • While treatments are largely effective, a significant risk of mortality remains.
  • The number of rejection treatments is a critical factor influencing patient survival post-transplant.
