Background:

  • Whiplash-associated disorders (WAD) frequently lead to work disability, but previous research presents heterogeneous findings.
  • Factors like age, gender, psychological status, and work type have been anecdotally linked to persistent work disability post-whiplash.
  • Limited sample sizes and varied methodologies in prior studies hinder definitive conclusions.

Purpose of the Study:

  • To examine the long-term work disability consequences of neck pain following motor vehicle accidents (MVAs).
  • To identify symptom-related and work-related factors associated with persistent work disability after MVAs.
  • To inform targeted interventions for mitigating work disability in post-whiplash patients.

Main Methods:

  • A prospective cohort study involving 879 individuals with neck complaints after MVAs who filed insurance claims.
  • Participants completed questionnaires at baseline (Q1), 6 months (Q2), and 12 months (Q3) post-accident, assessing injuries, complaints, work status, and disability.
  • Statistical analyses were performed to identify predictors of work disability at 1-year follow-up.

Main Results:

  • Over half (58.8%) of the studied population experienced work disability 1 year after the accident.
  • Independent predictors of 1-year work disability included older age and impaired concentration complaints at 1 month post-accident.
  • Physical complaints and work characteristics did not significantly predict long-term work disability.

Conclusions:

  • Age and concentration difficulties are significant independent predictors of sustained work disability following MVAs.
  • The type of manual labor (blue-collar vs. white-collar) and educational level were not associated with persistent work disability.
  • Interventions focusing on cognitive complaint recovery may be more effective for reducing work disability than those solely addressing physical symptoms.
