Purpose of the Study:

Main Methods:

  • Acute ischemic stroke patients (n=200) underwent multidetector computed tomographic angiography (MDCTA).
  • Aortic arch and ascending aorta were assessed for atherosclerotic plaques (presence, thickness, multiplicity, morphology).
  • Patients were classified using the TOAST criteria into subgroups: large-artery atherosclerosis (LAA), small artery occlusion (SAO), cardioembolism (CE), multiple potential causes (MPC), and cryptogenic stroke (CS).

Main Results:

  • Overall, 49.7% of patients had aortic atheromas.
  • Prevalence varied significantly by subgroup: LAA (80%), MPC (61.5%), SAO (50%), CE (44.7%), and CS (20.5%).
  • Plaques were predominantly mixed-type (93%), with 19% being ulcerated; subgroups differed in plaque multiplicity and thickness (>1mm).

Conclusions:

  • Aortic atheroma is common in stroke patients, especially in LAA, SAO, and MPC subgroups.
  • The presence of atheroma, particularly with soft components, highlights its significance in stroke etiology.
  • Findings suggest a link between aortic plaque burden and specific stroke mechanisms.
