Summary

The BRAF V600E mutation is more frequent in serrated colorectal adenocarcinoma and traditional serrated adenoma than in non-serrated adenocarcinoma and villous adenoma. This suggests a distinct serrated neoplasia pathway in colorectal cancer development.

Tumor Progression

Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.
Colon cancer is one of the best-documented examples of tumor progression. Early mutation in the APC gene in colon cells causes a small growth on the colon wall called a polyp. With time, this polyp grows into a benign, pre-cancerous tumor. Further...

Area of Science:

  • Oncology
  • Molecular Biology
  • Genetics

Context:

  • Colorectal cancer (CRC) pathogenesis involves multiple pathways, including the traditional adenoma-carcinoma sequence and a distinct serrated neoplasia pathway.
  • The BRAF V600E mutation is a key driver in various cancers, and its role in serrated colorectal adenocarcinoma (Sca) is under investigation.

Purpose:

  • To determine the frequency of the BRAF V600E mutation in serrated colorectal adenocarcinoma (Sca).
  • To provide indirect evidence for a distinct serrated pathway in colorectal carcinogenesis.
  • To compare BRAF V600E mutation rates across different colorectal lesion types.

Summary:

  • BRAF V600E mutations were detected in 36.4% of Sca and 26.7% of traditional serrated adenoma (TSA) samples.
  • No BRAF V600E mutations were found in non-serrated adenocarcinoma (NS-ca) or villous adenoma samples.

  • BRAF V600E mutation frequencies were significantly higher in Sca and TSA compared to NS-ca and villous adenoma (P = 0.0084).
  • Impact:

    • Findings support the existence of a separate serrated neoplasia pathway in colorectal cancer, distinct from the traditional adenoma-carcinoma sequence.
    • The study highlights the importance of BRAF V600E mutation analysis in understanding serrated colorectal lesions.
    • This research contributes to the molecular classification of colorectal tumors and may inform future diagnostic and therapeutic strategies.
