Fibromyalgia syndrome (FMS) affects over a third of primary headache patients, particularly those with frequent tension-type headaches. Increased headache frequency, anxiety, and poor sleep are linked to FMS in these individuals.

Background:

  • Fibromyalgia syndrome (FMS) is a chronic pain disorder with widespread pain and tenderness.
  • Primary headaches are common neurological conditions managed in specialized centers.
  • The co-occurrence of FMS and primary headaches requires further investigation.

Purpose of the Study:

  • To determine the prevalence of FMS in patients with primary headaches.
  • To identify clinical features associated with FMS in this patient group.
  • To explore the relationship between headache characteristics and FMS comorbidity.

Main Methods:

  • A cohort of 217 primary headache patients was assessed.
  • Standardized questionnaires evaluated FMS, headache features, anxiety, depression, disability, allodynia, and sleep quality.
  • Specific instruments measured fatigue and tender points in FMS patients.

Main Results:

  • Fibromyalgia syndrome (FMS) was diagnosed in 36.4% of primary headache patients.
  • FMS prevalence was higher in tension-type headache and patients with increased headache frequency.
  • Headache frequency, pericranial tenderness, anxiety, and sleep inadequacy were significantly associated with FMS.

Conclusions:

  • Fibromyalgia syndrome is highly prevalent in primary headache patients, especially those with frequent headaches.
  • Anxiety and sleep disturbances may contribute to central sensitization, linking FMS and headaches.
  • Targeting anxiety and sleep issues could be beneficial for patients with comorbid FMS and headaches.
