Background:

  • Diabetic retinopathy is a leading cause of vision loss, often associated with neovascularization and complications.
  • Central retinal vein occlusion (CRVO) can cause severe visual impairment and secondary glaucoma.
  • Previous ocular surgeries, including vitrectomy and cataract extraction, can alter the ocular environment.

Observation:

  • A 35-year-old male with diabetes presented with absolute hemorrhagic glaucoma.
  • The patient's eye was enucleated due to complications following CRVO, vitrectomy, retinal detachment repair, and cataract extraction.
  • Pathological examination revealed hematopoietic islands of erythroblastic precursor cells within the iris.

Findings:

  • The presence of erythroblastic precursor cells with mitotic activity in the iris is a rare pathological finding.
  • These hematopoietic islands suggest extramedullary hematopoiesis or a similar process occurring within the ocular tissues.
  • The distorted iris morphology was noted in conjunction with these cellular inclusions.

Implications:

  • This finding expands the understanding of potential ocular manifestations in advanced diabetic disease and severe ocular trauma.
  • It suggests that the eye, under specific pathological conditions, may exhibit hematopoietic activity.
  • Further research is warranted to elucidate the mechanisms and clinical significance of such intraocular hematopoietic findings.
