Background:

  • Intrathoracic goiters are enlarged thyroid glands extending into the chest.
  • They can present with compressive symptoms, mimicking lung malignancies.
  • Accurate diagnosis can be challenging due to atypical presentations.

Observation:

  • A 71-year-old woman presented with dyspnea and stridor.
  • Imaging revealed a mass compressing the right lung, with pleural and pericardial effusions.
  • Thyroid scintigraphy showed no uptake in the mass, and biopsies were inconclusive.

Findings:

  • Surgical exploration and histological examination confirmed a degenerated benign intrathoracic goiter.
  • The mass was compressing the upper right lung near the left atrium.
  • Associated effusions were noted on computed tomography.

Implications:

  • Intrathoracic goiter should be considered in the differential diagnosis of lung tumors, especially in older patients with compressive symptoms.
  • Non-invasive imaging and biopsy may be inconclusive, necessitating surgical intervention for definitive diagnosis.
  • Timely surgical management can resolve compressive symptoms and prevent complications.
