The Oxfordshire Community Stroke Project (OCSP) and Trial of ORG 10172 in Acute Stroke Treatment (TOAST) classifications showed moderate to excellent intra-observer reliability but only fair to good inter-observer reliability for minor stroke assessment.

Background:

  • The Oxfordshire Community Stroke Project (OCSP) and Trial of ORG 10172 in Acute Stroke Treatment (TOAST) classifications are standard tools for major ischemic stroke assessment.
  • Their reliability in classifying minor stroke, particularly in outpatient settings, requires further investigation.

Purpose of the Study:

  • To evaluate the intra- and inter-observer reliability of the OCSP and TOAST classifications for minor stroke.
  • To assess the consistency of these diagnostic tools among physicians with varying experience levels.

Main Methods:

  • Four physicians independently classified minor stroke cases using clinical data from 90 patients.
  • Kappa statistics were used to quantify both intra- and inter-observer agreement for OCSP and TOAST classifications.

Main Results:

  • Intra-observer reliability for both OCSP and TOAST ranged from moderate to excellent (kappa = 0.48-0.83).
  • Inter-observer reliability was good for OCSP (kappa = 0.64) but only moderate for TOAST (kappa = 0.42).
  • Neither classification demonstrated consistent reliability between observers.

Conclusions:

  • The findings suggest that the OCSP and TOAST classifications may have limited validity for minor stroke in typical outpatient populations.
  • Variable observer expertise could contribute to the observed inconsistencies in classification reliability.
