The Retinoblastoma Gene01:20

The Retinoblastoma Gene

Tumor suppressor genes are normal genes that can slow down cell division, repair DNA mistakes, or program the cells for apoptosis in case of irreparable damage. Hence, they play an essential role in preventing the proliferation of damaged cells.
The first-ever tumor suppressor gene called Rb was identified in retinoblastoma - a rare eye tumor in children. In inherited forms of the disease, a child inherits one defective copy of the Rb gene, which predisposes them to retinoblastoma. However,...

Summary

This study reports a rare case of multiple clear cell trichoblastoma lesions in a child, a neoplasm with follicular differentiation. The lesions showed incomplete regression, a rare phenomenon for this type of tumor.

Area of Science:

  • Dermatopathology
  • Pediatric Oncology

Background:

  • Clear cell trichoblastoma is a recently described, rare neoplasm with follicular differentiation.
  • Previous reports documented only solitary lesions, making multiple occurrences exceptionally uncommon.

Observation:

  • This case presents multiple clear cell trichoblastoma lesions in a pediatric patient.
  • Histopathology revealed dermal neoplasms connected to the follicular infundibulum, composed primarily of clear cells with specific nuclear and cytoplasmic features.
  • Immunohistochemistry showed positivity for AE1/AE3 and negativity for various markers, aiding in differential diagnosis.

Findings:

  • The diagnosis of multiple clear cell trichoblastoma was supported by clinical and histopathological findings.
  • Incomplete regression of the lesions was observed over a 20-month follow-up period, which is unusual for neoplasms with follicular differentiation.

Implications:

  • This case expands the understanding of clear cell trichoblastoma, highlighting its potential for multifocal presentation in children.
  • The observation of incomplete regression warrants further investigation into the behavior and management of this rare tumor.
