Summary

Patients with heart failure (HF) have a significantly higher risk of cognitive impairments (CI) compared to healthy individuals. Myocardial infarction severity is linked to memory deficits, suggesting ischemic cardiomyopathy contributes to CI in HF patients.

Background:

  • Limited data exists on cognitive impairments (CI) in chronic heart failure (HF) patients.
  • This study aimed to describe CI prevalence, type, and severity in community-dwelling HF patients.

Purpose of the Study:

  • To compare cognitive function in chronic HF patients versus matched community controls.
  • To identify factors associated with cognitive impairment in HF.

Main Methods:

  • A case-controlled study matched 50 HF patients with 50 healthy controls on age, gender, and IQ.
  • Neuropsychological tests assessed memory, attention, executive function, and reaction times.
  • Logistic regression analyzed the association between HF and CI.

Main Results:

  • 46% of HF patients exhibited mild to severe CI, compared to 16% of controls.
  • Cognitive impairment was independently associated with HF (OR=4.47).
  • CI severity correlated with learning deficits, previous myocardial infarction (MI), and number of MIs.

Conclusions:

  • HF patients have over a 4-fold increased risk of CI compared to controls.
  • Ischemic cardiomyopathy, indicated by MI severity, is a significant factor in developing CI.
  • Findings align with CI prevalence in end-stage HF patients awaiting transplantation.
