Balloon-assisted enteroscopy provides a safe and effective method for endoscopic retrograde cholangiopancreatography (ERCP) in patients with altered gastrointestinal anatomy. This technique facilitates access for various pancreaticobiliary interventions, demonstrating significant diagnostic and therapeutic potential.

Background:

Conventional endoscopic retrograde cholangiopancreatography (ERCP) poses challenges in patients with surgically altered gastrointestinal anatomy.
Balloon-assisted enteroscopy offers a novel approach for accessing the papilla of Vater or biliodigestive anastomosis in post-surgical patients.

Purpose of the Study:

To systematically review the effectiveness of balloon-enteroscope-assisted ERCP in patients with modified gastrointestinal anatomy.

Main Methods:

A systematic literature search was conducted across major databases (MEDLINE, Current Contents, PubMed) for studies on balloon-assisted enteroscopy for ERCP in patients with prior intestinal surgery.
Inclusion criteria focused on full-text English articles detailing diagnostic and therapeutic pancreaticobiliary interventions.

Main Results:

Sixteen reports involving 63 patients with diverse anatomical configurations were analyzed.
Successful interventions included balloon dilation, sphincterotomy, bile stone extraction, and stent placement, with no reported serious complications.
Both single and double balloon enteroscopy techniques were utilized.

Conclusions:

Balloon-assisted enteroscopy is a safe and feasible technique for achieving biliary or pancreatic access in patients with surgically altered anatomy.
The diagnostic and therapeutic utility of balloon-assisted ERCP is substantial and may be enhanced by specialized accessories.
