Background:

Anterior communicating artery aneurysms are common cerebrovascular lesions.
Coil embolization with adjunctive balloon remodeling is a standard treatment.
Delayed complications require thorough investigation.

Observation:

A patient presented with acute paralysis eight days post-aneurysm treatment.
Imaging revealed an infarction in the left anterior cerebral artery territory.
Angiography identified a migrated coil loop as the cause.

Findings:

The coil loop migrated from the aneurysm into the left A1-A2 junction.
This migration led to occlusion of the left anterior cerebral artery.
The patient experienced a significant neurological deficit due to the infarct.

Implications:

Delayed coil migration is a rare but serious complication of balloon-assisted coiling.
This case highlights the importance of post-procedure surveillance.
It suggests potential risks associated with adjunctive balloon remodeling techniques.
