Summary

Percutaneous cystolithotripsy (PCCL) combined with transurethral resection of the prostate (TURP) is faster and safer than transurethral cystolithotripsy (TUCL) for large bladder stones. PCCL reduces operative time and trauma compared to TUCL during simultaneous TURP.

Background:

  • Benign prostatic hyperplasia (BPH) frequently coexists with large bladder stones.
  • Management of coexisting BPH and large bladder stones requires effective surgical strategies.

Purpose of the Study:

  • To compare transurethral cystolithotripsy (TUCL) and percutaneous cystolithotripsy (PCCL) when performed concurrently with transurethral resection of the prostate (TURP).
  • To evaluate the efficacy and safety of TUCL versus PCCL in patients with prostate hyperplasia and large bladder stones.

Main Methods:

  • A comparative study involving 63 patients with prostate volume >40 cc and aggregate stone size >2.5 cm.
  • Patients underwent either TUCL (n=38) or PCCL (n=25) during simultaneous TURP.
  • Stone removal utilized pneumatic lithotripsy for TUCL and fragmentation via a suprapubic sheath for PCCL.

Main Results:

  • Mean operative time for stone removal was significantly less in the PCCL group.
  • The TUCL group experienced higher rates of residual stones and urethral stricture formation.
  • Operative time for TURP was similar between the two groups.

Conclusions:

  • PCCL offers a faster and safer approach for managing large bladder stones concurrently with BPH treatment.
  • The smaller working channel of TUCL leads to prolonged operative times and increased trauma.
  • Combined TURP and PCCL is recommended as a superior alternative to combined TURP and TUCL.
