Background:

  • Assessing decision-making capacity is crucial for understanding the impact of MS on daily functioning.

Purpose of the Study:

  • To longitudinally evaluate decision-making capacity in patients with early-stage multiple sclerosis (MS) using the Iowa Gambling Task (IGT).
  • To investigate the relationship between changes in decision-making and other clinical, cognitive, and behavioral factors over a two-year period.

Main Methods:

  • A cohort of 70 MS patients with mild disability (EDSS ≤ 2.5) underwent IGT assessment at baseline and a two-year follow-up.
  • Cognitive functions (memory, executive functions, attention), behavior, handicap, and perceived health status were also assessed.
  • Standardized change scores were computed to analyze performance dynamics.

Main Results:

  • IGT performance significantly decreased from baseline to retest (p = .005).
  • Worsening in IGT performance correlated with decreased perceived health status and emotional well-being (p = .05).
  • No significant association was found between IGT decline and relapse rates, disability progression, or cognitive/behavioral changes.

Conclusions:

  • Decision-making capacity can decline in MS patients over time, even with minimal neurological disability.
  • This decline may manifest as an isolated deficit, impacting perceived health and emotional well-being.
  • Longitudinal assessment using the IGT provides valuable insights into the subtle functional changes in MS.
