Summary

Surgical resection of sternal tumors followed by mesh and flap reconstruction effectively controls local disease. This approach demonstrated zero post-operative mortality in a study of eleven patients.

Surgical Fixation of Sternal Fractures: Preoperative Planning and a Safe Surgical Technique Using Locked Titanium Plates and Depth Limited Drilling

Background:

Sternal tumors, both primary and metastatic, present significant challenges in surgical management.
Chest wall defects following sternal tumor resection require robust reconstruction to ensure respiratory function and structural integrity.

Purpose of the Study:

To evaluate the efficacy of wide sternal tumor resection and subsequent reconstruction using prosthetic mesh and musculocutaneous flaps.
To assess the oncological outcomes and functional results of this combined surgical approach.

Main Methods:

Eleven patients underwent sternal tumor resection and immediate chest wall reconstruction.
Reconstruction involved prosthetic materials (polytetrafluoroethylene [PTFE], polypropylene, titanium) and pedicled musculocutaneous flaps (pectoralis major, latissimus dorsi, rectus abdominis).

Main Results:

No paradoxical respiration or post-operative mortality was observed.
One patient required prosthesis revision due to chest wall failure; one mesh was removed post-operatively.
Local recurrence occurred in one patient; five patients died from distant metastases.

Conclusions:

Wide resection of sternal tumors achieves good local disease control.
Reconstruction with mesh and musculocutaneous flaps is an effective method for managing large chest wall defects after sternal tumor resection.
