Background:

  • Hot flashes, excessive sweating, and fatigue are common symptoms that can significantly impact quality of life.
  • Endocrine dysfunction can manifest with a variety of nonspecific symptoms, necessitating thorough evaluation.

Observation:

  • Three patients presented with long-standing symptoms of hot flashes, excessive sweating, and fatigue.
  • Initial laboratory evaluations, including endocrine assessments and oral glucose tolerance tests, did not reveal a definitive cause for their symptoms.

Findings:

  • All three patients exhibited hyperinsulinemia despite normal glucose tolerance test results.
  • Treatment with metformin (500 mg twice daily) resulted in marked symptom relief for all patients.

Implications:

  • Hyperinsulinemia, even without hypoglycemia, may trigger a sympathoexcitatory response causing symptoms like hot flashes and sweating.
  • Metformin's potential sympathoinhibitory actions could offer a therapeutic avenue for managing these symptoms in patients with hyperinsulinemia.
