Mesalazine, a common inflammatory bowel disease drug, can rarely cause serious kidney damage (interstitial nephritis). Regular kidney screening is crucial for patients taking 5-aminosalicylate compounds to detect this complication early.
Area of Science:
- Nephrology
- Gastroenterology
- Pharmacology
Background:
- 5-aminosalicylate compounds, including mesalazine and sulfasalazine, are standard treatments for inflammatory bowel diseases.
- Mesalazine-induced interstitial nephritis is a rare but severe adverse effect of this therapy.
- High individual morbidity associated with mesalazine-induced nephritis underscores the need for vigilance.
Observation:
- A 15-year-old male patient with a history of idiopathic proctocolitis was diagnosed with mesalazine-induced nephritis.
- The patient presented with symptoms indicative of kidney damage secondary to mesalazine treatment.
- Renal function was monitored during the course of treatment.
Findings:
- The patient exhibited interstitial nephritis attributed to mesalazine therapy.
- Prompt initiation of corticotherapy led to a favorable clinical response.
- Renal function showed improvement following the introduction of steroid treatment.
Implications:
- This case highlights the importance of vigilant renal screening in patients receiving 5-aminosalicylate therapy.
- Physicians should consider interstitial nephritis in patients on mesalazine presenting with renal dysfunction.
- Corticosteroids may be an effective treatment for mesalazine-induced interstitial nephritis.
