Summary

A rare fibrous pseudotumor encasing a testis was identified in a 43-year-old male. This benign paratesticular lesion can mimic malignancy, necessitating careful differential diagnosis.

Area of Science:

Background:

  • Fibrous pseudotumors are uncommon benign paratesticular lesions, second only to adenomatoid tumors.
  • They represent approximately 6% of paratesticular lesions and are often reactive to trauma, hydrocele, infection, or inflammation.

Observation:

  • A case of diffuse fibrous pseudotumor/fibromatous periorchitis is presented in a 43-year-old male.
  • The lesion completely encased the right testis and was adjacent to a hydrocele cavity.

Findings:

  • Fibrous pseudotumors, while uncommon, are a recognized cause of paratesticular masses.
  • These lesions have a peak incidence in the third decade but can occur at any age.

Implications:

  • Clinically, fibrous pseudotumors can mimic testicular malignancy, potentially leading to unnecessary radical orchidectomy.
