Tumor Progression

Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.

Summary

This case study details a rare biphasic pulmonary blastoma (BPB) in an elderly man, highlighting its aggressive nature and rapid progression. Early detection and surgical intervention were insufficient to prevent recurrence and eventual mortality.

Background:

  • Biphasic pulmonary blastoma (BPB) is an exceptionally rare primary lung malignancy.
  • This tumor type exhibits aggressive behavior and a propensity for rapid progression.

Observation:

  • A 75-year-old male presented with a large, well-defined tumor in the right upper lobe, identified via CT scan.
  • CT-guided biopsy confirmed malignant cells with sarcomatous components.

Findings:

  • Surgical resection (right upper lobectomy, lymph node dissection, chest wall resection) was performed for pathological stage IIB (T3N0M0) BPB.
  • Histopathology revealed immature mesenchymal and epithelial elements resembling embryonic lung.
  • Tumor recurrence in supraclavicular lymph nodes occurred within two months post-surgery.

Implications:

  • This case underscores the aggressive nature of biphasic pulmonary blastoma, even after aggressive treatment.
  • The rapid progression and early recurrence suggest limited efficacy of current therapeutic strategies for advanced BPB.
  • Further research into novel therapeutic targets and treatment modalities for BPB is warranted.
