Calcium is not only the most abundant mineral in bone but also the most abundant mineral in the human body. Summary

A rare parathyroid carcinoma, initially misdiagnosed as a benign thyroid adenoma, caused metastatic disease and hypercalcemia. Surgical removal of metastases normalized calcium levels, highlighting the importance of accurate diagnosis.

  • A 47-year-old woman presented with unexplained weight loss and hypercalcemia.
  • Imaging revealed bilateral pulmonary metastases.
  • Histology of a metastasis indicated an endocrine tumor with low parathyroid hormone expression.

Findings:

  • The patient was diagnosed with functioning parathyroid carcinoma metastasized eight years after being labeled with a benign thyroid adenoma.
  • Surgical resection of all metastases was performed.
  • Post-surgery, serum calcium and parathyroid hormone levels normalized.

Implications:

  • This case underscores the diagnostic challenges in parathyroid carcinoma.
  • Surgical resection is the primary treatment modality.
  • Effective management is crucial to prevent mortality from hypercalcemia.
