Summary

Gliosarcomas, a rare glioblastoma subtype, present with mixed glial and sarcomatous features. These rare brain tumors have a poor prognosis, similar to glioblastoma multiforme, with a median survival of 7 months.

  • Review of clinical data, including age, sex, tumor location, and treatment.
  • Histopathological examination including glial fibrillary acid protein (GFAP) and MIB-1 proliferation index.
  • Pre-operative magnetic resonance (MR) spectroscopy in two cases.
  • Main Results:

    • Median patient age was 63 years (range 52-79); 11 males and 5 females.
    • Tumor locations varied, with the frontal lobe being the most common site (n=6).
    • Median survival time was 7 months (range 2-11 months).
    • Histology confirmed glial cells positive for GFAP within sarcomatous tissue.
    • MIB-1 proliferation index ranged from 20% to 70%.
    • MR spectroscopy revealed lactate peaks, indicative of necrosis and hypoxia.

    Conclusions:

    • Gliosarcoma remains a rare and aggressive brain tumor with a poor prognosis.
    • Pre-operative MR spectroscopy and tumor characteristics like location near the dura and inhomogeneous contrast enhancement may suggest gliosarcoma.
    • Adjuvant chemotherapy studies are warranted to improve outcomes for patients with gliosarcoma.
