Background:

Talectomy is a surgical procedure involving the removal of the talus bone.
Its long-term outcomes, particularly after trauma without tibiocalcaneal fusion, remain under-documented.
Assessing the efficacy of talectomy in pediatric trauma patients is crucial for understanding long-term functional recovery.

Observation:

A 10-year-old patient underwent primary talectomy due to trauma.
The patient was followed for an extended period of 60 years post-surgery.
Minimal symptoms were reported throughout the 60-year follow-up period.

Findings:

Primary talectomy for trauma, without subsequent tibiocalcaneal fusion, demonstrated positive long-term results.
The procedure facilitated a good quality of life for the patient over six decades.
