Background:

  • Palifermin, a keratinocyte growth factor, treats oral mucositis in hematopoietic stem cell transplant (HSCT) patients.
  • Known side effects include rash, pruritus, and erythema.
  • This study investigates a specific cutaneous reaction post-palifermin therapy.

Observation:

  • A patient developed erythema and lichenoid papules in intertriginous areas after HSCT and melphalan, followed by palifermin.
  • Biopsies showed verruca-like lesions, negative for human papillomavirus.
  • Increased keratinocyte proliferation was noted via Ki-67 and cytokeratin 5/6 staining.

Findings:

  • Palifermin therapy can lead to papular eruptions resembling lichen planus or warts.
  • Intertriginous erythema is also a potential adverse reaction.
  • Histologic findings showed features of verruca plana.

Implications:

  • These palifermin-induced skin reactions may not require treatment and can resolve spontaneously.
  • Ki-67 and cytokeratin 5/6 staining can aid in identifying palifermin-related adverse cutaneous events.
