Summary

Hypopituitarism (HYPO) in elderly patients presents with non-specific symptoms often mistaken for aging. Prompt diagnosis and hormone replacement therapy can significantly improve quality of life and reverse debilitating conditions.

Background:

  • Symptoms are frequently misattributed to aging or other comorbidities.
  • Early recognition is crucial as symptoms can be life-threatening.

Observation:

  • Two elderly patients with distinct HYPO presentations were analyzed.
  • Case 1: Pituitary macroadenoma causing HYPO, leading to cognitive and gait improvements after treatment.
  • Case 2: Empty sella causing global anterior HYPO, presenting with severe hyponatremia, pancytopenia, and rhabdomyolysis, resolving with treatment.

Findings:

  • Hormone replacement therapy (cortisone acetate, L-thyroxine) significantly improved cognitive function, gait, and overall well-being in both patients.
  • Treatment of HYPO reversed severe biochemical abnormalities including hyponatremia, pancytopenia, and rhabdomyolysis.
  • Effective management of HYPO in the elderly can lead to substantial clinical benefits.

Implications:

  • Elderly patients presenting with non-specific, debilitating symptoms should be evaluated for HYPO.
  • Timely diagnosis and treatment of HYPO can significantly enhance the quality of life for older individuals.
  • This study highlights the importance of considering HYPO in the differential diagnosis of age-related decline.
