Orbscan pachymetry offers better repeatability than ultrasound for measuring corneal swelling caused by extended wear contact lenses. This finding is crucial for understanding central and peripheral corneal changes during contact lens wear.

Area of Science:

  • Ophthalmology
  • Biomedical Engineering
  • Corneal Physiology

Background:

  • Contact lens (CL)-induced corneal swelling is a common concern during extended wear (EW).
  • Accurate measurement of corneal thickness is essential for detecting and managing CL-induced changes.
  • Comparing different pachymetry methods is vital for clinical and research applications.

Purpose of the Study:

  • To compare the repeatability and agreement of ultrasound and Orbscan pachymetry.
  • To assess these methods in detecting central and peripheral corneal swelling.
  • To evaluate measurements after extended wear of high and low oxygen permeability contact lenses.

Main Methods:

  • Corneal thickness was measured at five locations (central, peripheral) using ultrasound and Orbscan.

  • Measurements were taken before and after 1 week of extended wear with two types of CL (lotrafilcon A, etafilcon A).
  • 20 subjects with normal ocular health participated in the study.
  • Main Results:

    • Orbscan pachymetry demonstrated superior repeatability and agreement compared to ultrasound, especially in peripheral measurements.
    • Ultrasound showed higher coefficients of repeatability than Orbscan in both swollen and non-swollen corneas.
    • High oxygen permeability lenses (lotrafilcon A) induced less corneal swelling than low oxygen permeability lenses (etafilcon A).

    Conclusions:

    • Orbscan pachymetry is more repeatable than ultrasound for measuring corneal thickness changes.
    • Orbscan is a valuable tool for studying central and peripheral corneal swelling variations during contact lens extended wear.
    • These findings aid in selecting appropriate pachymetry techniques for contact lens research and clinical practice.
