Management involves complex decisions regarding revascularization (in situ vs. extraanatomic), graft material selection (e.g., Dacron, ePTFE, homografts), and stent graft indications.

Impact:

Open surgery for IAAs has variable early mortality (11-36%) and significant late complication rates (3-14%).
Stent grafts offer lower early mortality (around 5%) but higher rates of late events (20% at 15 months).
Prognosis after stent grafting is poor with persistent infection, but favorable in stable patients with controlled infection.
