Peritoneal fluid contains various adipokines, with concentrations influenced by body mass index, inflammation, and underlying diseases. These findings suggest a new research area for visceral adipokines in gastrointestinal conditions.

Background:

  • Visceral adipose tissues secrete adipokines, but their presence in peritoneal fluid was unknown.
  • This study investigated novel (cartonectin, omentin) and classical adipokines (leptin, adiponectin, resistin, visfatin) in ascites patients.

Purpose of the Study:

  • To determine peritoneal fluid concentrations of various adipokines in patients with ascites.
  • To explore correlations between adipokine levels and clinical parameters.

Main Methods:

  • Enzyme-linked immunosorbent assay (ELISA) and Western blot were used to measure adipokines.
  • Ninety-six patients with ascites, predominantly due to liver cirrhosis, were included.

Main Results:

  • All measured adipokines were detected in ascites fluid with wide variations.
  • Adipokine levels correlated with body mass index (BMI), inflammation (local and systemic), and serum concentrations.
  • Resistin, leptin, visfatin, and omentin showed specific correlations with peritonitis, underlying disease, white blood cell count, C-reactive protein, and ascitic albumin.

Conclusions:

  • Peritoneal fluid adipokine concentrations are individualized and influenced by peritonitis, BMI, and systemic inflammation.
  • These findings highlight the potential role of the peritoneum and visceral adipokines in gastrointestinal diseases.
  • This research opens new avenues for understanding adipokine function in peritoneal fluid.
