Summary

T-cell prolymphocytic leukemia (T-PLL) is an aggressive lymphoid disorder. Novel treatments like purine analogs and alemtuzumab improve survival, but stem cell transplant remains the only cure.

Area of Science:

  • Hematology
  • Oncology
  • Immunology

Background:

  • T-cell prolymphocytic leukemia (T-PLL) is a rare lymphoid malignancy.
  • T-PLL is characterized by aggressive clinical course and poor response to chemotherapy.
  • Median survival for T-PLL patients is historically short.

Purpose of the Study:

  • To review the current understanding of T-cell prolymphocytic leukemia.
  • To discuss novel therapeutic approaches and their impact on patient outcomes.
  • To highlight the role of allogeneic hematopoietic progenitor-cell transplantation in T-PLL management.

Main Methods:

  • Review of existing literature on T-cell prolymphocytic leukemia.
  • Analysis of treatment outcomes with purine analogs and alemtuzumab.
  • Evaluation of hematopoietic progenitor-cell transplantation as a curative option.

Main Results:

  • Purine analogs and alemtuzumab have improved response rates and survival in T-PLL.
  • Responses to current treatments are often transient.
  • Allogeneic hematopoietic progenitor-cell transplantation is the only potentially curative treatment for T-PLL.

Conclusions:

  • While new therapies improve outcomes, T-PLL remains a challenging malignancy.
  • Allogeneic stem cell transplantation offers the only curative potential for T-PLL.
  • Exploring nonmyeloablative transplantation is crucial for older T-PLL patients.
