Summary

Laparoscopically monitored colonoscopic polypectomy (LMCP) offers a safe and effective solution for difficult-to-remove polyps. This combined approach demonstrated no recurrence in long-term follow-up, ensuring good oncologic outcomes.

Background:

Colonoscopy is standard for benign polyp removal.
Difficult polyps pose challenges due to size, base, or location.
A novel approach combining laparoscopy and colonoscopy was investigated.

Purpose of the Study:

To evaluate the long-term follow-up of laparoscopically monitored colonoscopic polypectomy (LMCP).
To assess the oncologic safety and effectiveness of this combined approach.

Main Methods:

Patients undergoing LMCP from May 1990 to January 2008 were analyzed.
Prospective follow-up included colonoscopies at 6 months, 1 year, and annually thereafter.

Main Results:

209 polyps were removed from 160 patients (mean age 74.7 years).
Mean follow-up was 63.37 months (range 6-196 months).
No polyp recurrence was observed during the follow-up period.

Conclusions:

The combined endoscopic-laparoscopic approach is safe and effective for difficult polyps.
Malignant lesions identified during LMCP can be managed laparoscopically in the same procedure.
This strategy ensures good long-term oncologic outcomes and avoids repeat interventions.
