Summary

Prehydration with Voluven significantly improved kidney function during laparoscopic surgery compared to lactated Ringer solution. This suggests Voluven is effective in protecting renal function in patients undergoing these procedures.

Background:

Laparoscopic surgery involves pneumoperitoneum, which can negatively impact renal function.
Maintaining adequate hydration and renal perfusion is crucial during surgical procedures.
Hydroxyethyl starch solutions are used for fluid resuscitation, but their effects on renal function require specific evaluation.

Purpose of the Study:

To compare the efficacy of prehydration with hydroxyethyl starch 130/0.4 (Voluven) versus lactated Ringer solution in preserving renal function during laparoscopic cholecystectomy.
To assess the impact of different prehydration strategies on intraoperative diuresis and creatinine clearance.

Main Methods:

A randomized single-blind clinical trial was conducted on 29 patients (ASA 1-2) undergoing laparoscopic cholecystectomy.
Patients received either 500 mL of lactated Ringer solution or Voluven as prehydration 30 minutes before surgery.
Intraoperative fluid management, anesthesia protocols, blood pressure, diuresis, and creatinine clearance were monitored and recorded.

Main Results:

The Voluven group demonstrated significantly higher intraoperative diuresis (1.71 mL/kg/h vs. 0.47 mL/kg/h; P = .017).
Creatinine clearance was also significantly higher in the Voluven group (176.44 mL/min vs. 61.90 mL/min; P = .036).

Conclusions:

Prehydration with Voluven is an effective strategy for protecting renal function during laparoscopic surgery.
Voluven may mitigate the adverse renal effects associated with pneumoperitoneum.
