Background:

  • Computerized adaptive testing (CAT) item banks require updates to maintain accuracy and relevance.
  • New items must be rigorously linked to existing CAT systems before implementation.

Purpose of the Study:

  • To evaluate 41 pretest Daily Activity (DA) items for integration into an operational CAT system.
  • To assess the impact of new items on the performance of the DA-CAT.

Main Methods:

  • 6,882 patients with various impairments participated in the study across 147 clinics.
  • 41 new DA items were administered alongside the existing DA-CAT-1 in five waves.
  • Real data simulations compared scoring consistency, test information function (TIF), person standard errors (SEs), and content range between the original and new CAT (DA-CAT-2).

Main Results:

  • 29 of 41 pretest items were retained.
  • DA-CAT-2 demonstrated improved scoring consistency (ICC = 0.90 vs. 0.96) compared to DA-CAT-1.
  • Improvements in TIF and person SEs were observed for patients with higher DA functioning, and ceiling effects decreased from 16.1% to 6.1%.

Conclusions:

  • Item response theory (IRT) and online calibration methods proved effective in enhancing the DA-CAT.
  • The integration of new items improved the overall performance and precision of the DA-CAT system.
